SUPPORT FACILITATOR
This position will be of interest to individuals who are looking to make a difference in the lives of the people we support by promoting opportunities for individual growth in all aspects of a person’s life, ensuring dignity and respect.
The Support Facilitator will
- Support individuals in achieving their full potential by facilitating relationships, skills, and roles which promote people’s participation as valued members of their community
- Work in partnership with other team members and families in supporting people to realize and achieve their life goals
- Provide personal support, including medication administration and assisting people with personal care
- Compile and maintain up to date records and communications related to individuals and their progress
- Be available to work regularly scheduled shifts, including evenings and weekends
Knowledge and Skills
- Understanding of supporting individuals with complex care needs in a sensitive and respectful manner
- Thorough knowledge and understanding of the concepts and objectives of the person centered planning process
- Ability to work as part of a team
- Strong written and verbal communication skills
- 2 years related experience
Education and Training
- Two years post secondary education and/or related work experience
- Valid CPR/FA certification
- Knowledge of community resources
Assets
- Valid Ontario driver’s license and willingness to operate a van
- Hours: flexible
- Shifts: days, evenings and weekends (days & evenings)
- Compensation: hourly
Ongoing Training
Ongoing training sessions in areas such Medication Administration, CPR/First Aid, Person-Centered Planning
